A copy of the State Birth Certificate should accompany this form. A copy can be emailed to ballyfermotassumption@gmail.com, or left into the sacristy.
One of the parents should be a Roman Catholic (in cases involving legal guardians please talk to the priest).
Minimum requirement is that there be one Godparent. If there are two they must be male and female.
Godparents must be over 16rs old, Roman Catholic, and have made their First Communion and Confirmation.
The signature of the mother alone is sufficient when she is single, is sole guardian and is not requesting that the father's name be entered.

If people are able, an offering of €50, to cover expenses and church upkeep, is suggested. But any offering, or none, is fine too. It can be offered on the day.
