Health Professions Advising Committee: open your file and request an advisor for 2023-2024
IMPORTANT - If you are submitting this request form after the deadline of October 6th, in addition to submitting this form, please email Professor Cahill directly at mcahill@holycross.edu

If you would like to apply to medical, dental, veterinary or optometry school with committee support in the next application cycle, you need to fill out this form to start the process, even if you have filled it out in the past.

You also need to have a signed waiver in your file to start collecting recommendation letters. If you have not already, sign the Health Professions File Waiver Form and email it to healthprofessions@holycross.edu before you submit this form. If you have done this in the past, you do not have to submit this again - we have the waiver and previous recommendation letters in your file.

Allied health programs like PA, NP,  PT, etc. do not accept committee recommendations. If one of these programs is your goal , it does not make sense to open a file and request a committee advisor. However, our office is happy to advise and support you in your application process in other ways - assessing competitiveness, discussing requirements, offering advice, helping with application essays, preparing for interviews, etc.You may set up a meeting with an advisor by requesting an appointment here.
