This is the 2023 form for current & new TIMBERWOLVES.

The deadline to re-register is March 1st.

Please remember, that any adult that will be volunteering, or present at activities and events will need to register too. Background checks are done every 2 years.

2. For new registrations, print out a YOUTH medical release & liability waiver. Bring it to the next meeting. Leaders will have copies of the release forms at meetups too.
