This form collects information that we need to list your item in the auction catalog and to acknowledge your generous donation. (Your email and phone will only be used if we need to contact you about your item information and will not be listed in the donor acknowledgement.)

With the current recommendations and restrictions due to the pandemic, try to adjust your donations to meet current guidelines: consider outdoor or patio activities allowing for recommended distancing; consider offering restaurant/business/ organization certificates; deliveries, services.....etc.

Submit one form per item.

You will receive two emails after you submit this form. One will acknowledge your submission and the second will be asked to reply-all with a photo or image of your item. This will be displayed in the auction catalog.
