Thank you sincerely for participating in the 2021 Stan Whitlock Solo Competition. Please carefully read all questions and thoroughly answer before submitting. If a mistake is made, applicants may simply resubmit the form with all of the correct information and notify the competition chair. Applications with a timestamp after midnight on December 15, 2021 will not be considered (NO EXCEPTIONS).

Link to UNEDITED, PUBLIC UNLISTED YouTube video. Evidence of editing will result in immediate disqualification. Performance must be identical to what would be presented in the semi-final round. *
There is a $65 application fee before application is complete. If this fee imposes a financial burden, please reach out to the chair of the competition. Incomplete applications at the deadline of December 15, 2021 will not be considered. Applicants can pay via PayPal, noting the applicant's name in the "notes" section or via e-mail.
