This form is to be used by all students, faculty, and staff in the MPH community when any accident occurs that results in injury. The form can be submitted by a teacher or supervisor on someone's behalf, or by an individual if they have suffered an injury on the MPH campus or at an MPH-sponsored event off-campus. When submitted, the form will be reviewed by the individual most appropriate to do so (Health Office, Human Resources, etc.).
