OUR AGREEMENT

Please read carefully. You must agree to the terms below (by checking each box) to participate in Fall Festival 2019.

I understand that my club is responsible for the following: *

Set up, Booth Volunteers throughout the Event, Clean up crew

1 Table, and at least 1 chair for my booth workers (PTSA will provide 1 tablecloth)

A booth sign (min. 20"x30" poster board) AND Game Instructions/Menu Items/Price etc

All needed items for my activity

A sponsor, a parent or other adult to assist during set-up and take-down

I understand that if my booth is found to be unmanned during any portion of the event, we may be assessed a 10% penalty on our profits

I understand that if my booth does not participate in the set-up and clean up of the event, we may be assessed a 10% penalty on our profits. This is a team effort and we need everyone to work together!

I understand that my organization will receive $0.80 for each ticket sold and must remit $0.10 per prize ticket redeemed.
