I give permission for my child named above to participate in the Summer Day Camp of St. Paul’s Evangelical Lutheran Church, June 20th-June 23, 2023 and the field trips on 6/21, 6/22, and 6/23. I authorize the staff and volunteers of St. Paul’s to provide for medical care in case of an emergency. I understand that St. Paul’s will not be responsible for property brought to the program by my child. I give permission to St. Paul’s and the Minneapolis Area Synod to use photographs of my child in its publicity materials.
