Please complete in order to be considered for admittance to Mu Alpha Theta, Tuscarora High School, Chapter #4169. Please read ALL directions carefully in each section and answer all questions honestly. Please read the following and select your response before moving on.

By acknowledging that you meet the necessary qualifications for acceptance into Mu Alpha Theta, you attest that you have (1) completed at least one semester at Tuscarora High School, (2) completed of at least two high school math courses, which can include Algebra I and Geometry taken in middle school, (3) maintained a 3.0 GPA in all high school math courses and overall. If you do not yet meet the qualifications, please exit the application and look to apply next year or whenever you have met the qualifications. *
