Operation Change San Diego will begin the 18 Week community based behavioral change program on Saturday May 4th from 9am-12pm at The Salvation Army Kroc Center (6753 University Ave., San Diego, CA 92115).  
The program is for women age 45+, experience at least two comorbid conditions (obesity, heart disease, type 2 diabetes) and whose conditions are exacerbated by limited mobility due to joint pain. 
Operation Change San Diego is offered in Spanish and is designed to educate Hispanic women affected by healthcare disparities in our community.  
Space in the program is limited, Please answer the questions below to register for the program. A member of the Operation Change team will contact you to confirm your registration has been received and answer any questions you have.
