Dear Parent/Caregiver

Please complete this form if your child will continue to learn from home from Wednesday 17th November.

If you have more than one child enrolled at Sherwood School, please complete one form for each child.

This will ensure we know your child is safe at home when we take the attendance each day and that we have enough home packs for those who need one.

There is no need to notify us should you decide to send your child to school at a later date.
