This agreement ("Agreement") is entered into between the undersigned individual ("Client") and Ewers and Mine, Inc., the owners of Sandbox, collectively referred to as the "Service Provider."

Description of Services: The Services provided under this Agreement include 5 individualized sessions per month (within a 30-day period). Each session has a duration of 20 minutes. Unused sessions cannot be carried over to the following month. Sessions cannot be scheduled back-to-back on the same day but can be scheduled on the same week or spaced out on the same day. The sessions will be conducted one-on-one with the facilitator.

Facilitator's Role: The facilitator will collaborate with the Client to understand their goals and tailor each session to address specific needs. The facilitator will share tools and skills to assist the Client in their personal development.

Guest Participation: Clients are allowed to invite one other person to each session if desired.

Session Format: All sessions will be conducted virtually, and the Client must have access to the internet or phone. Sessions will be scheduled by the Client using the online calendar scheduler provided by the Service Provider. Monthly calendars will be released at the beginning of each month.

Payment: The cost for the service is $200 per month. Payments will be collected through Zelle or Venmo. This is a month-to-month subscription service with no long-term commitment. Clients can choose to continue for the following month (with 5 more sessions) or take a break. Rejoining after a break is subject to availability, as spots are limited.

Spot Reservation: Upon signing this Agreement and submitting payment, the Client's spot for the month's sessions will be secured. As this constitutes a subscription service, subsequent monthly participation will not necessitate re-submission of this form; only the payment process is required. In the event of a break and subsequent return, re-submission of the form will be necessary to recommence the service.

Confidentiality: The content of the sessions is confidential and will not be shared with third parties. However, it is important to note that this service is not conducted under a privacy-protected platform.

Not a Replacement for Therapy: The sessions provided under this Agreement are not a form of therapy or a replacement for therapeutic services.

By signing below, the Client acknowledges their understanding and agreement to the terms outlined in this Agreement.
