CDI Reservation Agreement              
RESERVATION REQUEST:
Submission of this form is NOT confirmation of a reservation; confirmation is required. Communication will be made by email or Google calendar invite. CDI Staff will review your responses and follow up with you within 3-5 business days; Requests should be made Monday-Friday and a minimum of 5-7 days prior to the start of your event  to ensure enough time for staff to respond

RESERVATION EXPECTATIONS:
The User must ensure that all equipment, including the special event carts, and all areas used are clean and restored to the condition in which they were found. Tables, chairs, and floors must be cleared, and all garbage must be removed from the premises immediately following the event.

RESERVATION CANCELLATIONS:
To cancel a reservation, please email cdi@depauw.edu with the name and date of your event at least 24 hours before the expected start time.
