Please fill out the below form for a winter dispensation request 2024.

A Panel of 3 people comprising of CHA advisory committee members and/or CHA staff make dispensation decisions.

Please note - Weekly dispensation requests close every Wednesday at 12pm and club secretaries/school contacts will be notified by 5pm Friday of the decision. Canterbury Hockey will not be held accountable if the above Wednesday 12pm timeframe is not meet to provide a decision on the request. Full season/long term dispensation requests will be assessed on a case by case basis and it may therefore take longer for a decision to be made.

If you do have a late request please fill out the form and then email sam@canterburyhockey.org.nz stating you have submitted a late request.

All requests and decisions are made public on the CHA website.
