CHCL Rink Rental Agreement
This information is being collected in accordance with the Digital Privacy Act and will be used for the purposes of renting a Community League Ice Rink.  Information collected will be retained for a period of 60 (sixty) days after rental.  Information may be shared with members of the Edmonton Police Service or Alberta Health Services if required as a result of their attendance at the Community League Ice Rink with respect to this event but is protected by the privacy provisions of the Digital Privacy Act.  If you have questions about the collection, use of disclosure of the personal information provided on this form, contact THE COMMUNITY LEAGUE, or the Edmonton Federation of Community Leagues.

All rink rentals require third party liability insurance in the amount of $2,000,000.00, naming Cameron Heights Community League as an additional insured. Our address is: PO Box 78118, RPO Callingwood NW, T5T6A1 *
All rink rentals will require a damage deposit cheque in the amount of $200 provided to the league representative at the time of rental. This will be returned at the end of the rental provided no damages to the rink. *
