Application form to become a member of the Epiphyte Educational Society (EES), the non-profit society that governs the Vancouver Island School of Innovation and Inquiry (VISII). The Society's incorporation number is S0073178.

Membership is approved by the Board of Directors. To be in good standing, each Member pays annual dues of $5 per year. Members in good standing are voting Members of the Society and are entitled to vote at general meetings.
