Letter of Recommendation Request
Instructions: If you would like me to write a letter of recommendation, please submit this form at least 2 weeks in advance of the deadline, but preferably earlier.

Some of these questions involve logistics, while others ask for more personal details about yourself. Include things even if you think I already know/remember the answers. I have many students and I don't want to leave anything out. The answers to these questions will help me write the best letter possible - I want you to get the job/internship/REU!

Note: If for any reason I believe I cannot write you a positive letter of recommendation, I will notify you as soon as possible.

What is your name? *
In what context do I know you? (If you've taken courses with me, please include course, semester, and grade, anything noteworthy from our course.) *

Why are you applying? What experience or interests do you have that make you a good candidate? *
When is the deadline for the letter? (Note: Feel free to remind me of deadlines via email. If you haven't heard confirmation from me, then I likely haven't sent in your letter.) *
Academic achievements: Tell me about your grades (GPA), major, academic strengths, and weaknesses. Suggest how your academic background has prepared you for the position for which you are applying. Include anything special or unique about your academic background. *
Nonacademic background: Please tell me about your jobs, hobbies, sports, community work, political or social involvements, travel, etc.
What do you want to do after you graduate from Grinnell? (It's OK if you aren't sure)
Primary academic and work traits: Please list five. If you can't describe yourself, think about how a colleague or professor would describe you. *
Primary personal and social traits: Please list five. If you can't describe yourself, think about how a friend would describe you or else ask friends to describe you.
Include your program application - a draft is fine. This might be a cover letter, research statement, etc. If there is no relevant document, skip this question.
