If you attended Module #7 on 04/15/2021 but:

(1). You were not signed in using your own information and login details
                                                         OR
(2). Joined the training as part of a team or on behalf of someone else (in person)

We need you to sign in here indicating your participation in the Module, which will allow you to receive a certificate of attendance for today. This attendance certificate will be emailed from Oakland Schools to your email address and can be used for your records or district needs.

Please use this form (which is separate from the SCECH/SW Sign-in form) to indicate your attendance today. This will allow Oakland Schools and START to capture your attendance that may not be recorded using the Zoom Attendance Report.
