Consensus Gradwrite (Ottawa) Registration Form
This is the official registration form for the Consensus Gradwrite tournament held at Glebe CI on Saturday, October 26th. Please fill this form out once per school.

How many working buzzer sets are you able to bring? (Each set gives your school a $10 discount, up to a maximum discount of $30 per team). If we have enough already and don't need more, we will contact you before the tournament to let you know.) *

Please read and acknowledge the following terms.

1. The base entry fee for this tournament is $80 per team before applying discounts. Payment can be made via e-transfer (contact@consensustrivia.com), cash, or cheque (made out to Consensus Trivia), anytime on or before the tournament date.

2. The Guaranteed Registration deadline for this tournament is Saturday, October 12th. Any teams that register on or before that date are guaranteed spots. We may or may not be able to accommodate teams that register after that date.

3. The Late Drop deadline for this tournament is Saturday, October 12th. Any teams that drop out after this date will still have to pay 50% of their registration fee.

4. Teams must be accompanied by an adult supervisor at all times during the tournament, preferably a staff member.
