I acknowledge the following:

All vendors will need to be approved by the FFOA event committee

If accepted, art related vendor fees are $150 per 10' x 10' space, to be invoiced after acceptance

If accepted, non profit art related vendor fees are $25 per 10' x 10' space, to be invoiced after acceptance

If accepted, my 10 x 10 tent will be weighed down with at least 10 lbs. per leg/ 40 lbs. per tent

If accepted, my designated 10x10 area will be clear of any garbage or debris

Vendor Waiver

I release the Falls Festival of the Arts, Falls Art at Large, Art Guild of Menomonee Falls, Village of Menomonee Falls, Menomonee Falls Downtown Inc, MF Business Improvement District and all its agents and representatives including the Village of Menomonee Falls from any liability whatsoever for loss, damage, theft or defacement of any property displayed or used in connection with my display at the Falls Festival of the Arts event, nor will I be a party to any legal action against the  Falls Festival of the Arts event and Falls Art at Large and all its agents and representatives including the Village of Menomonee Falls.
