In an attempt to simplify and streamline the meeting sign-in process we are asking everyone to pre-register their email addresses and phone numbers so we can identify participants without having to ask repeatedly for people to identify themselves (this has been challenging for people joining on the telephone).

Please indicate if you plan to join on the computer alone, phone alone, or phone and computer. Please provide your phone number even if you plan to join on the computer alone - in case you end up joining on the phone due to technical difficulties.

If more than one voting member is joining as part of a group and using a single phone line or signing in on the same email account they should each fill out their own form.
