What are your thoughts of an Integrated Health Hub?

With a shortage of primary care providers and a growing population, we want to make sure all North Toronto residents have access to team-based primary care. To do this, we are establishing an “Integrated Health Hub” in Mount Pleasant West and Englemount-Lawrence Heights. Integrated Health Hubs bring together health and community services into the same physical space. This will provide patients, their families, and caregivers with easy access to a range of support from different service providers. It also helps health and service providers communicate with each other and coordinate care.
