If you have multiple schools that need to receive your transcript, you only need to complete this form one time.  You will be given the opportunity to list all schools that need to receive your transcript.

Please note: You do not need to complete this form for transcripts that are sent via Common Application, SENDedu, Coalition, or CFNC.
