CLI Client Survey - 2020
CLI wants your comments all year, but we also send out at least 1 formal survey each year to collect ideas about what individual clients like, dislike, and/or wish for.

Be honest.  CLI gets better when we know what we should do more of-- AND what we should do less of-- AND what maybe we should immediately stop doing.

Every box on this survey will require an answer, but anything you type in will allow you to move on to the next question.  However, just remember we all learn better when someone takes the time to teach us.

CLI was awarded two grants from the Ohio Department of DD to help us become a more innovative provider.  Your answers to the following questions will help us with these grant projects.  DD services in Ohio were changing even before COVID shut workshops down this year.  This means that we need to build new ways to support our clients.  As CLI develops new services, we need to know what is most important to YOU!
