This form is for Kindergarten new student to the district enrollment only.  Please make sure all phone numbers have area codes and in the following format (XXX) XXX-XXXX.  Names should consist of first and last name.  It is important that your student's information is correct.

1. Birth Certificate
2. Social Security Card
3. Immunization Records
4. Proof of Residency in your name-one of following
    1. Utility bill (water or electric)
    2. Lease Agreement
    3. 2019 Property Tax
    4. Mortgage Statement
5. Copy of Parent/Guardian Drivers License
