Community Health Hub Feedback Form


Community health hubs are health and wellness resource delivery centers located in trusted community-run spaces. What draws community members to trusted community spaces is each other. Community-run spaces are trusted because they are operationalized by community members who understand the experiences and environments of the people they serve. 


The CHH model operates within collective action, drawing on the power of connection between organizations, systems, and communities. Through cross-sectoral partnerships, we can work together to identify and bring health and wellness resources into community-run spaces that are socially, geographically, and physically accessible. 


The CHH model is meant to be responsive and dynamic to community needs. It relies on feedback and direction from our partners. Please let us know how to work together to grow and improve the community health hubs.
