1. If you have multiple students, a separate form for each child must be filled out.
2. Billing will occur at the end of each month and will be processed through the SMART Tuition system. You can use the same payment method as the one currently used to charge tuition payments OR you can enter a new payment method like a credit or debit card.
3. If you wish to change your order after submitting the form, please contact the office (
office@daviswaldorf.org).
4. Changes to your order must occur before 3:00pm on the Wednesday before the pizza is served.
