YCM Child Registration Form 2023-2024

Hello and welcome!

Here at Rye Lane Chapel, our YCM (Youth & Children's Ministry) offers children and teens a safe space to learn about Jesus whilst building friendships and having fun. Through music, craft, games, activities, tech and discussion, our young people learn from the Bible about walking in faith with our awesome God who absolutely loves them.

We invite you to join us!
Please complete this form to register your child/ren with us (complete one form per child).

Good to Know:
  • YCM sessions run during most Sunday morning church services
    (service start time - 10:30am)
  • Ages 3-16 (children under 3 can attend but must be accompanied by an adult throughout)
  • All are welcome, including those with additional needs or from different religious backgrounds
  • Any personal information you provide through this form is kept secure and accessed only by YCM/Church Leaders on a need-to-know basis
  • All YCM team members are DBS-checked and have a heart for children, and we diligently follow Safeguarding Procedures as prescribed by the Baptist Union
  • If you prefer to complete a paper registration form, or require any assistance, please be in touch (details below)
  • By completing this form, you register your child with YCM at Rye Lane Chapel and consent to them attending sessions under the care of our team

Important NEW Class Dismissal Information:

- Nursery & Primary School Children: All Nursery and Primary School aged children MUST be collected by an adult* at the end of YCM sessions immediately after service finishes.

- Secondary School Children: Children in Secondary School leave their group's room by themselves at the end of YCM sessions.

*This must be a parent/guardian recognised by YCM Teachers or specified on the child's Registration Form. Older siblings (who are under 18) can only collect their younger sibling(s) if no parent/guardian is present AND if this has been pre-arranged and communicated to the child's YCM Teachers.
