2023-2024 Club Wildcat Request Form Information & Instructions below for the upcoming school year. Club Wildcat will begin on Monday, August 7, 2023.

Club Wildcat after-school program is for Kindergarten through 6th grade students. It is designed to provide enrichment opportunities as well as a safe environment for students to attend after school. Club Wildcat begins when the last bell for the afternoon rings each school day, and ends at 5:15 p.m. Club Wildcat will operate under the Scottsboro City School calendar for 2023-24. First day of Club Wildcat will begin on Monday, August 7, 2023. Elementary students (Nelson & Caldwell) will be bused to Collins Intermediate School where Club Wildcat will be housed. Collins Intermediate students will walk to Club Wildcat at the end of the school day. Parents' may pick up their child anytime between the closing of regular school day and 5:15 p.m. in the back of Collins Intermediate School. If your child receives a snack or is counted in attendance, you will be charged for that day as attending Club Wildcat.

1. Club Wildcat availability will be determined by space and staffing availability. Completing this form does not guarantee a spot into the CLUB WILDCAT after-school program.

2. Parent/Guardian must complete the Enrollment Request Form each new school year.

3. Completing this form below ONLY guarantees that your child will be put on a CLUB WILDCAT enrollment request list. Completing this enrollment request doesn't secure your child a spot into the Club Wildcat after-school program.

4. Families who request enrollment with multiple children will be considered. If one child of the family has been accepted into the Club Wildcat program, all children within that family will be accepted into the Club Wildcat program. However, they must be listed on the enrollment request form.

5. Confirmation phone calls will be made to applicants who are accepted into Club Wildcat by Carrie Bradford. Once you have received acceptance into the program, a formal application will need to be competed and a paid registration fee of $30.00 (non-refundable) per child, will secure your child(s) spot for the 2023-24 school year. Applications process will be further explained once and/or if accepted into the Club Wildcat program.

6. If you register your child for CLUB WILDCAT and your child does not attend a single day that week, you will be charged a minimum fee per week, per child.  Once you register your child, you agree to attend the CLUB WILDCAT program a minimum of a part-time basis. There will be a minimum weekly fee for your child to attend regardless of whether he/she is in attendance or not. You will be billed regardless of whether your child attends one day (part-time) or (full-time).

7. Club Wildcat parent contract will be required in acceptance of terms and agreement as well as financial responsibility. Refusal to sign contract will void any attendance or acceptance into Club Wildcat.

8. The right to attend SCS Club Wildcat may be revoked for flagrant and/or persistent violations of the district's Code of Conduct. CLUB WILDCAT will follow the Scottsboro City Schools Code of Conduct when dealing with all disciplinary issues. The same rules apply in after school as they do during the regular school day.

9. All school fees & Club Wildcat fees must be up-to-date, registration fee, parent financial & parent agreement contract signed,  code of conduct, and student release information must be signed once accepted into the Club Wildcat program.

10. Student attendance and disciplinary records from previous school year will also be reviewed before acceptance into the Club Wildcat program.

11. Parent/guardian with previous year attendance into Club Wildcat will be considered and reviewed for good standing.

12.Club Wildcat will operate on the Scottsboro City School 2023-24 calendar. Early dismissals for weather etc. will be determined and notified to parents/guardian on a case by case basis.

13. The 2023-24 Club Wildcat after school activities will follow the guidance from the Alabama Department of Health (ADPH), the Center of Disease Control (CDC), and other federal and state recommendations concerning best practices. While we cannot, nor can anyone, anticipate every challenge that may lie ahead for us in 2023-24 school year it is or goal to carry out clear, actionable steps that can be implemented by our faculty, staff, administrators and students. It is our goal to return to as normal as possible for the 23-24 school year.

14. If you do not receive a phone call, your child or children will automatically be listed on a waiting list for availability throughout the 2023-24 school year as it becomes available. Carrie Bradford will contact you as positions become available.

15. To discontinue or withdraw your child from attending Club Wildcat, you must submit a note in writing to Carrie Bradford explaining the last day your child will attend the Club Wildcat program, one week prior to the change. If you need to make changes to your child's registration form, it must be done in person and by the legal guardian of the child. If you become one month past due with paying your fees, you will be notified that your child will not be allowed to attend the program until fees are paid.

16. If there is a change in custody, the parent/guardian must give a copy of all documentation to Carrie Bradford at Scottsboro Board of Education.

2023-24 Club Wildcat Fees & Billing Requirements

Part-time is considered 1 or 2 days a week
Full-time is considered 3 days or more a week.

Registration Fee $30.00/per child (non-refundable) once accepted into Club Wildcat

1. The fee for Club Wildcat (part-time) 1 child/per day is $15.00/day

2. The fee for Club Wildcat (full-time) 1 child/3 days or more a week is $65.00 week.

3. If you register your child for Club Wildcat and your child does not attend a single day that week, you will be billed a minimum charge of $30.00 per week, per child. Once you register your child, you agree to attend the Club Wildcat program a minimum of a part-time basis. There will be a minimum weekly fee for your child to attend regardless of whether he/she is in attendance or not.

4. The part-time and/or full-time rate will cover your child's daily snack and any supplies used for enrichment services during Club Wildcat. Additional fees may be needed based on circumstance of project/activity.

5. Parent/guardian may choose to pay weekly, monthly or bi-weekly. Payment option will need to be indicated on the the official registration form once accepted into the Club Wildcat program.

6. Student Payments-Please make all checks payable to Scottsboro City Board of Education.  Place your check or cash in an envelope, mark it CLUB WILDCAT and include your child’s name, school and amount enclosed on the outside.  It is very important that this information be on the outside of the envelope. All bookkeeping for CLUB WILDCAT is completed at the Central Office-not at the local schools.  Please have your child turn their envelope in to the school’s office or their classroom teacher on Monday morning.  If you are not sure how many days your child will be attending each week, please pay on the last day of each week your child attends.  CLUB WILDCAT teachers do not accept fee money.

7. Monthly Invoices will be available for pick up at Club Wildcat. If you need balance information please contact Carrie Bradford at Scottsboro Board of Education cbradford@scottsboroschools.net or (256) 218-2100.

8. A $30.00 charge will be made on all returned checks.  If a check is returned, you will be notified to pick up the check.  From that time on, no checks from you will be accepted. You will have to pay cash.
