You were directed to this form due to your residency being outside of the South Holland boundaries per the Southwest Midget Football League (SWMFL). Prior to proceeding with your interest in the South Holland Jets Youth Organization (SHJYO), we must follow the boundary guidelines outlined by the Southwest Midget Football League.

The SWMFL requires all participating organizations to direct interested families to the organization within their boundaries to provide an opportunity for that organization to provide information about their program(s). Upon receiving information from the home organization, families can proceed with their organization of choice. Should you choose the SHJYO, you will need to complete a Boundary Release Form which will be provided to you upon you notifying SHJYO of your decision.

You will be able to proceed with registration up on receipt of the completed Boundary Release Form.
