I hereby authorize Aaron’s Presents, its directors, officers, employees, volunteers, and agents to photograph, video record, and voice record the above child in association with the above child’s participation with Aaron’s Presents. I hereby grant to Aaron’s Presents all rights, title, and interest of every kind to use my child’s first name, photograph, video and/or voice recording for the purpose of publicizing/promoting my child, my child’s project(s) for or with Aaron’s Presents, and/or Aaron’s Presents and its purpose and mission. I understand that last names of children under 18 will not be included on Aaron’s Presents materials unless consent is specifically requested and given by me or another parent/guardian. *

I understand that Aaron’s Presents does not provide liability or medical insurance coverage for my child or my child’s possessions while participating in this program. This includes any property damage or personal injury to a third party that my child might cause, personal injury to my child, or property damage to my child’s or my property as a result of participating. I agree that I am responsible for arranging and paying for any insurance to cover such events. *For participants doing projects at Hidden Pond Farm ("Farm") in Derry, NH, this clause also applies to the Farm.

To the fullest extent provided by law, I hereby release, discharge, and hold harmless Aaron’s Presents, its directors, officers, employees, volunteers, and agents, past, present, and future, from any and all liability, claims, damages, demands, obligations, actions, causes of action whether based in law or equity, medical or other expenses, including, but not limited to any claims for death, disability, or loss of services, comfort, or society, or any other losses of any kind which may arise as a direct or indirect result of participating in Aaron’s Presents programs and activities, including, but not limited to, any liability, claims, damages, demands, obligations, actions, causes of action whether based in law or equity, medical or other expenses, including, but not limited to, any claims for death, disability, or loss of services, comfort, or society, or any other losses of any kind that relates to any transportation which is provided by Aaron’s Presents, its directors, officers, employees, volunteers, and agents, past, present, and future, for my child. *For participants doing projects at Hidden Pond Farm ("Farm") in Derry, NH, this clause also applies to the Farm. *

I hereby authorize Aaron’s Presents, its directors, officers, employees, volunteers, and agents to provide transportation for my child in association with my child’s participation with Aaron’s Presents. I understand that permission from a parent/guardian will be obtained by email, phone or text before my child is transported by an Aaron’s Presents employee, Board Member or volunteer. *

I understand that the terms of this Agreement are contractual in nature. I further understand that the terms contained herein constitute the entire agreement and may not be orally modified. Specifically, but not limiting the foregoing, no claim of waiver, modification or consent with respect to any provisions of this document shall be made except on the basis of a written instrument executed by or on behalf of the parties to this Agreement. This Agreement shall be governed by and interpreted in accordance with and under the laws of the Commonwealth of Massachusetts. This instrument shall be interpreted fairly to achieve its purpose as if drafted jointly by the parties hereto. *

All of the above permissions will be granted for the duration of my child's involvement with Aaron's Presents, which may extend past this year and include my child’s participation in Aaron’s Presents Givers & Visionaries, Agents of Change, or other programs. *For participants doing projects at Hidden Pond Farm ("Farm") in Derry, NH, this clause also applies to the Farm. *
