Please read this information carefully before beginning. We are beginning to prepare for the upcoming school year and are requesting feedback from parents regarding student classroom placement. All classroom placements occur through collaboration among a student's current teachers, school counselor and school administration. The team takes into account several different factors when determining class placement. We would like to receive input from parents as we begin class assignments for next year. Please keep the following items in mind when filling out this form:
Individual teacher requests are NOT accepted; however, parents may request specific teaching styles that work best for their child. You may include the name of one teacher that you do NOT feel would be a best fit for your child. If you prefer that your child not be placed with a specific teacher, please be clear about your reasons why (these will be kept confidential) Again,

In some cases it
may be possible for siblings to have the same teacher; however, if staff members and administration believe that it is in the best interest, academically and socially, of the siblings to placed in different classrooms, we will separate them.
This request is for homeroom assignment ONLY. Students may have different reading group teachers throughout the year. Please note that students in 4th - 6th grade will have all teachers at that grade level.
Administration will
review discipline data in relation to student-to-student conflict when determining
placement of students for next school year.
Once class list are made, there will be no changes. We are asking for your input in advance so we can avoid any issues. If you wish for us to consider any special factors, please complete the form below by April 15. You do not need to submit this form unless you are providing specific feedback regarding your child.
Please understand we will take into consideration your requests and concerns. While we can not guarantee that all requests will be honored, we will work hard to integrate them into your child's placement.
