The Complex 2023-2024 Application Form
Before you fill out this form, please read through the 2023/24 Performing and Creative Arts Class Schedule - September 11, 2023 - June 14, 2024 (available on our website and at the front desk). The registration fee is $25 for a single student or $40 for a family. You will receive an emailed invoice to pay this fee within a few days after we receive your completed form. Please fill out the application form below, we will then get back to you with confirmation and acceptance information. If you have more than one student, please fill out separate forms for each. 
In the next few days you will receive a return email from The Complex with confirmation of enrollment, tuition rate, important info, and a link to set up your automatic payment.  September tuition and a credit card on file is due at time of registration to hold your place in classes; a link will be sent via Square. September tuition is non-refundable as it holds the spot in class. All subsequent tuition installments are due on the first calendar day of each payment month. Thank you!

I understand that I am responsible for the full session tuition (date of registration through June 2024), as I am reserving a space in class by signing this registration form for the remainder of the 2023/2024 season.  I understand that if I choose to withdraw from a program, I need to give 30 days written notice by writing a letter directed to Nichola Johnson. I understand that I will be charged for the following month's tuition after I submit the Cancellation Request and my student(s) will still be able to attend classes through the last month I have paid for. I understand after January 1 2024, no cancellation forms will be accepted and I am responsible for the full remaining tuition amount regardless of my child's participation.

Performance Agreement: I understand that performances/showings are an important part of The Complex curriculum and participation is mandatory unless I have spoken with a director and have been excused from the performance. I understand that additional rehearsals will be called outside of class time and my student is expected to attend unless their absence has been excused. I understand that the week leading up to theatre performances (aka Tech Week) is mandatory and rehearsals will likely take place every evening during that week. The Complex will make every effort to get rehearsal schedules out to parents and students as soon as possible.  *
PERFORMANCE INFORMATION: The Complex produces many productions per year.  Some students will participate in one and some in many.  The Performance Package is a one-time, non-refundable $100 fee charged to all students on January 1 and applicable for our Spring Student Performance in June. This fee will include 2 tickets to our Spring Student Performances/Gallery Shows in June, costume/accessory/prop rental, art supplies and materials, a performance t-shirt, and access to the recorded performance after completion. More information on this will be announced. There are no additional costume fees.  Additional tickets may be purchased.  The Performance Fee aids in the rental of venues, allows us to keep ticket prices low, and ensures there are no extra or surprising out-of-pocket fees for costuming, materials, rehearsals, etc.
