CircleYouth Covid-19 Agreement 2020/2021 – Circle Drive Alliance Church
Based on the Government’s guidelines and Circle Drive Alliance Chuch’s internal practice guidelines, we will include the following protocols and procedures for our CircleYouth gatherings. This agreement will only need to be signed once for the 2020/2021 school year. By typing the Parent's/Guardian's and the Participant's name below and selecting "I give permission", you are agreeing to the following statements from CircleYouth as a part of Circle Drive Alliance Church (CDAC):

1. Parents/Guardians are those having legal custody over the Participant;

2. Participants will not attend CircleYouth or its Activities if they show any symptoms of illness similar to those of Covid-19 including but not limited to fever, cough, sore throat, runny nose, or shortness of breath due to illness within 48 hours of the gathering start time;

3. Participants will not attend CircleYouth while waiting for Covid-19 test results themselves, if they have been in contact with someone waiting for test results, or observing required isolation because of testing;

4. If a Participant begins to show symptoms during a service, a Parent/Guardian will be available to pick up the Participant as soon as possible;

5. If a Participant has attended a CircleYouth gathering within two weeks prior of a new positive test for Covid-19 they will contact CircleYouth immediately for the purpose of contact tracing;

6. Participants will bring and wear a mask in common areas, where physical distancing might not be possible, and when requested to by CircleYouth leadership. Masks will be available to those who do not have one;  

7. Participants will use hand sanitization when entering or exiting the building or when otherwise requested to do so;  

8. Participants will be allowed to request to sit with up to two other Participants within their extended household/"bubble" that the Parents/Guardians of each Participant have all mutually agreed upon while registering for each event;

9. Participants will maintain two (2) metre physical distancing from those outside their extended household/“bubble” as best as possible, whether in a large group or a small group;  

10. Participants will minimize all physical contact including but not limited to hugs, high fives and fist bumps;
 
11. Participants will follow the facilities guidelines posted throughout the building including but not limited to directional flow, unavailable areas, and washroom capacity;  

12. Limited food or drink services will be provided by CircleYouth.  Participants will avoid sharing food and drinks during CircleYouth or its Activities and the following precautions will be taken by our team when serving/preparing food: Pre-packaged food and beverage on regular weekly gatherings. If there is food that is not pre-packaged, it will be served by volunteers. Utensils/tongs used to hand out drinks, bags, etc., Gloves worn by anyone handling/serving food, Mask is required when dealing with/serving food/beverage items. Students are allowed to bring their own snack/drink (Starbucks or Tim Hortons etc. but are responsible for proper disposal.)

13. Participants will register for each CircleYouth gathering and understand that not doing so may result in not being able to attend that event;  

14. Rather than assigned seating, students will have the freedom to choose their seat to sit in, however they must write their name on paper with the pen provided and place it on their chair. They must only use that chair for the remainder of the gathering in that room.;

15. Parents/Guardians will not be allowed to enter the building except in the case of an emergency and are to make arrangements for dropping off and picking up the Participant from the parking lot;

16. Parents/Guardians and the Participant understand that provincial guidelines and CDAC guidelines are there to mitigate the possibility of transmission of any illnesses but cannot guarantee that it will not occur;  

17. A copy of this document provided by way of facsimile transmission or electronic transmission shall be as binding upon the Parent/Guardians as an originally copy.
