Sequence Audit 
Welcome to the sequence audit application form. Please take a moment to complete the form so that I can hit the ground running on our planned audit day. There is the chance that we will encounter some authentication hiccups so please bear with me should access to your accounts be delayed slightly.

Sequence Audits are $250 CAD and payable via PayPal.
Add on: $50 CAD authentication services payable via PayPal.
All services are required to be paid in full at time of booking and are non-refundable.
Maximum 5 active sequences can be audited in one service session. If you have more than 5 sequences that you'd like audited, please let me know and we will adjust the rate accordingly.
You will receive a complete audit of each sequence and a list of next-steps to create a more effective sequence sales system.
Once you've completed this form, you will receive an email within 24-48 hours that will include your invoice, audit delivery date, any additional audit instructions and your contract. Weekends and holidays are excluded from this time frame.
