Book Request Form for Heritage High School
Please fill out this form if you want to check out books from the Heritage High School Media Center.

School librarians will be putting together Virtual Academy requests to be picked up DAILY during school hours. If you need to make special arrangements for hours or pick up, you will be able to do so at the bottom of this form.

If you are on campus and request books using this form, your books will be delivered to your first period teacher OR you will be emailed and books will be placed on a cart outside the Media Center.
