This form is for use by vendors who run keyset and/or keyboard GBs only.

Please use this form to:
1. Submit vendor information for a first time rating, if your business has never been rated before
2. Submit vendor information for an update, if you believe you meet the criteria for a new rating

The MK Trust and Safety Admin Team will review the information and contact you with any questions.

Whether you are a new vendor or existing vendor, we appreciate your effort to ensure that consumers and hobbyists are protected.
