After I receive this completed form I'll email you for a down payment on the retreat of 50%. This is non-refundable, though if something happens and you are not able to come, you will receive the payment as future credit toward another retreat. Upon arrival the other 50% is due at registration.

COST (includes 4.5 days of workshop instruction, one on one meetings, breakfast, lunch, afternoon snack)
$400 discount!! On reservations on or before 8/31: $800 ($400 due upon registration to hold your spot)
Purchased on or before 9/30: $1200 ($600 due upon registration to hold your spot)
These retreats are kept small and unfortunately we cannot reserve your spot until we receive the down payment.
