This form must be completed by the parent / guardian of all licensed children (anyone in the cast who is 15 or under up to and including 31 August 2023).

Important note

As a Licensed Child in EVITA your child will not be able to leave the rehearsal venue or Theatre Royal Wakefield in break-times, and must be collected at the end of the session by a responsible adult.

Please make sure they bring packed lunches and plenty of snacks with them as they will not be able to go out of the building. This is a legal requirement from the Licensing Authorities. For EVITA we will apply for a Body of Persons license for all child performers. For this, we need to know if your child will have taken part in any performances in the 6 months preceding EVITA (March to August 2024). Please tell us the name of the show, the performance dates, the venue (school/theatre/hall?) and number of performances or put 'none' if necessary. *
CAST BIOG for programme: About yourself (maximum two sentences). Please try to imagine that you are reading this back to yourself on the opening night! Don’t put ‘I’m looking forward to….’ Etc. as by the time the public read this you will already be on stage. Please write in first person (e.g ‘I have really enjoyed working on this show’). To help keep you safe, please do not include any personal information, like the name of your school. Theatre Royal Wakefield retain all editorial rights to these cast biogs *
