Please complete and return this form by Friday, February 14, 2020 to let us know your plans for the 2020-2021 school year.
You can do it online (this form) or using the paper version that was sent in your student's backpack on January 22nd, 2020 (and return it to your classroom teacher or building receptionist).

Fill out one paper or one electronic form per student.

Please note that if you have a current student and an incoming kindergartener or first grader, you must fill out a separate Enrollment Application form for K-1 students.
