Please fill out a separate form for each student (after you complete the form it will present you with the option to submit a new one). Currently The Conservatory is teaching online and in person lessons, our teachers are incredible at teaching through zoom and other video services, allowing the students to have an engaging, positive musical experience from home. We look forward to making great music with you!

One unique opportunity The Conservatory offers is Parent/Student joint lessons where the family can learn instruments or singing together. This has been an incredibly effective way to learn music and everyone ends up having a blast in the process. Working together often creates new opportunities for learning and an over all great experience. If you're interested let us know here.

Photo Release Waiver
The Conservatory PHOTO RELEASE FORM

I hereby grant The Conservatory permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of The Conservatory and will not be returned.

I hereby irrevocably authorize the The Conservatory to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge The Conservatory from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Cancellation Policy


Every student is allowed one emergency cancellation without penalty per year. An emergency cancellation is any cancellation within the 7 days preceding the scheduled lesson. After this if you cancel within the 7 days before the lesson you will be held accountable for the lesson price. You may cancel 7 days before a lesson without a penalty. On most holidays and breaks we remain open and do not assume the lessons are cancelled unless the student or the teacher inform us of the cancellation. We know everyone's schedule is busy and changing. We try to work with each situation be flexible and find a time for a make up if the schedule allows. By attending any lessons or classes at The Conservatory you agree to abide by the Attendance and Cancellation Policy.
