Reschedule Request Form
The League Office requests that Captains provide at least one week notice for all reschedule requests. 

Please be advised that the likelihood of a reschedule approval goes down drastically the less notice you give. Please give us as much time as you can for us to make adjustments to the schedule. 

Please keep in mind that submitting this form does NOT guarantee the office will be able to make your change, this merely provides us notification that you are requesting a change. 

Reschedule Request Approvals or Denials will be sent to the email you submit the form under. In the event of a Reschedule Denial, the league office will follow up with the reason for the denial.
