Kathryn Fa is certified by The Trauma Centre at JRI to facilitate Trauma Sensitive Yoga as an adjunctive intervention for Complex &/or Developmental Trauma. Supervision: Kathryn Fa’s clinical supervision is with Dave Emerson and/or Jenn Turner (Co-Directors, Centre for Trauma & Embodiment), within a monthly TCTSY peer reflection group. Consent Statement: Just like any form of physical activity, yoga carries a risk of injury and I agree to take precautions and responsibility for my own actions. I have, to the best of my ability, disclosed any necessary information about prior injuries, pre-existing medical conditions and personal history that may be relevant to my safety and the safety of others. I understand that TCTSY is an adjunctive practice and that participants are advised to have access to an established reflective space within which they can verbally & cognitively process their trauma history. I am aware that Facilitators of TCTSY do not provide psychotherapy during the course of the yoga session but that topics in psychoeducation may be covered. I understand that TCTSY is not considered useful if I have needed a hospitalisation within the last 6 months, for my trauma related experiences. By agreeing to work with Kathryn Fa I am agreeing to the information in the above section(s) being collected, processed and stored by Kathryn Fa such that she may comply with her legal and regulatory obligations to the IA-TCTSY Guidelines on Ethical Practice, the Information Commissioner's Office & in accordance with her Privacy & Confidentiality Policies. I understand that no information about me will be shared with any third party unless Kathryn Fa judges an exception to confidentiality has been met. A TCTSY Facilitator has the right to deny or terminate service to any person who may represent a threat to the safety of themselves and/or others. I have read and discussed the above information with Kathryn Fa and have been given the opportunity to ask questions and discuss any concerns about entering into a course of Trauma Sensitive 1-1 Yoga or TCTSY.
