HTPS 5th Grade Band & Orchestra Registration
All 4th grade families are asked to indicate whether or not their child is participating in band or orchestra next year.  We hope that you will join us for a year of fantastic music-making in our band or orchestra program!

Please fill out all fields below and click submit.  All information will then be accessible by our staff.  If you prefer, you may also print the form and submit it to your primary school's music teacher at least one week prior to the last day of school.
