- We have a spot or two available on a few of our teams this spring. Please fill out the form below so we can assess whether it makes sense to try to schedule a tryout for your son/daughter. You will not be asked to pay a tryout fee at this time. We will email you and let you know if a tryout makes sense.

PLAYER TYPE: What type of player is your son/daughter interested in being and what level of Commitment will you have to Blazers Basketball Club this coming season (defined as March - May)?
Please consider your response carefully. Our Tournament teams need a sufficient number of fully committed players on each team to make for a positive experience for the whole team, so we do factor this into our team formation and other decisions, and we do require players to honor their commitment.

Other Sport Commitments during March-May (if you chose Player Type 1 you may skip this question): Please provide information on what other "sports commitment(s)" you have in March - May and any schedule information you have about those (please be as specific as possible) (Ex: Town Soccer: practices Mon/Wed, games Sat afternoons). TEAM LEVEL: Within your town, what level of team is the player on this winter?   Notes:
- Please do NOT mention AAU/Club Teams
- a Recreational or "Town" team is where all players that sign up are placed on a team (no "cuts"), and the team is not part of the MetroWest Travel League or Central MA Travel League
- If the player is in Grade 6 or Below and playing on both a Recreational Team and a Travel Team, please respond to this question based on the type of Travel Team
- If the player is in 7th or 8th Grade, please respond solely based on the level of Travel Team they are playing on. If they are NOT playing travel but are on the "school team" for their Middle School, please choose 9: Other and state that in the comments for "Other".
- If the player is in Grade 9 or Above, please identify the  "school team" they are playing on (Options 6-8). If not playing on a school team but playing on a Recreational Team, please choose option 1. If not currently playing on a team, please choose 9: Other and state that in the comments for "Other".

New Player Background:
If your child played on a Blazers Club Team (grade 4 or above) in Spring 2023 or Fall 2023, simply type "prior player" and move to the next question.

For New Players, please share some important background on the player so we slot him/her into an appropriate initial tryout group by answering the following question: 

Relative to JUST the boys or girls that play basketball in your child's grade and in your town, please provide a CANDID assessment of where he/she stands in terms of height, speed/aggressiveness, and basketball skills (shooting, ballhandling, etc.) Please respond in the following format: Height: top 10%, Speed/Aggressiveness: bottom 25%, basketball skills: average). 
For example, if there are 40 girls on rec/travel teams in Town XYZ and 20 play travel, then only the "starters" on the A travel team would likely have basketball skills in the top 10% (top 4 out of 40 players).
Uniform Number Preferences (New Players Only):
If you are registering a player that is new to the Blazers, please specify his/her top 5 uniform numbers (in order of preference) so once we finalize the players on each team we can expedite the process of determining unique uniform numbers for the players. If you are going to include popular numbers like 0, 23, 30, 33, 35, etc. feel free to provide more than 5 options. If none of the numbers you provide are available for that gender/grade (or you do not provide a number) then we will assign a random number, so feel free to provide as many numbers as you like.

Terms and Conditions

General Terms: 
As the parent or legal guardian of the player/child named above, I hereby give my full consent and approval for my child to participate in this program. I understand that there are certain risks of injury inherent in the practice and play of this sport, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical or developmental disabilities or infirmities that would restrict full participation in these activities, except as listed above. I acknowledge that it is my responsibility to be knowledgeable about concussions in youth sports (CDC has a free course at https://www.cdc.gov/HeadsUp/index.html), and that Alpha Youth Sports/Blazers Basketball Club has provided me with a link to concussion-awareness education material. If my child has any concussion like symptoms or has sustained a head injury, I agree to visit a licensed health care professional for evaluation, and to not allow my child to participate again until cleared by the licensed professional. I understand that my medical insurance is expected to cover the cost of my child’s injuries. I grant permission to the coaches or volunteers involved with this program to seek medical care for my child in the event I cannot be reached. I agree not to hold Alpha Youth Sports, or any of the parties/facilities connected with this program, responsible for any injury or accident that may occur to my child (or anyone accompanying them) during the program, whether the result of negligence or any other cause.

Video Release: 
For good and valuable consideration, the receipt of which is hereby acknowledged, I hereby consent to the photographing of my child and the recording of my child's voice by Alpha Youth Sports, Inc. (“AYS”) during this Program. I understand that the term "photograph" as used herein encompasses both still photographs and motion picture footage. Provided that AYS does not identify my child's last name, nor town of residence, in these photographs or recordings, I irrevocably consent to the use of these photographs and/or recordings singularly or in conjunction with other photographs and/or recordings for advertising, publicity, commercial or other business purposes, as well as to the reproduction and/or authorization by AYS to reproduce and use said photographs and recordings of my child's voice, for use in all domestic and foreign markets. I hereby release AYS, and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies or other assigned third parties, their directors, officers, agents and employees from all claims of every kind on account of such use.

Other Terms:
- All tryout fees and program fees are non-refundable
- I agree to familiarize myself and my child with the rules associated with the facilities used in this Program, including their usage, behavior and other rules that apply to the player and the parents, siblings, and others that accompany the player
- I understand and agree that the availability of Tournament Teams and Trail Blazers Teams is dependent on the number of players by grade interested in these options, and that Alpha Youth Sports/Blazers Basketball Club may need to change my child's team, player group, or program in an effort to balance the considerations and desire for "fit" of BOTH my child AND the other participants in the program that are seeking to compete with/against players of a similar current skill level.
- I understand that if my child, or anyone accompanying them to a practice, game, or other event violates a facility rule, becomes a discipline problem, is unwilling to abide by the Program's rules/procedures, or is in any way risking the safety of (or negatively impacting the experience of) the other players in the program, my child may be dismissed from the Program, and that no money will be refunded.
