LCPS Community Meals: Summer 2021
**THIS FORM IS FOR LCPS FAMILIES AND STUDENTS**

Greetings #TeamLCPS students and families! We have GREAT NEWS! During Summer 2021, LCPS will be offering families weekly meals once again, FREE of CHARGE! Meal distribution will take place every Wednesday, from 9 a.m. - 11 a.m., at our four elementary schools. Distribution will begin June 16 and run through July 26. Families will receive a week's worth of breakfast and lunch for each child. This program is open to all children ages 2-18.

If you are unable to access any of our distribution sites during the specified times, we will once again be offering home delivery as well!

Please complete the following form and let us know if you are interested in participating in our summer meals program. Also let us know if you plan to pick up meals or are requesting home delivery.

**Please note that someone must be home to receive meals when they are delivered.**
