BSL - Sign-Up Sheet for LPGA Amateurs Team
Please complete all the information below to sign-up to play.  

The 5/16 match is AWAY AT CORAL SPRINGS

FOR AWAY GAMES:
You will pay to play at the Pro Shop at the Course - No pre-payment!  Please bring $5 for the game.

FOR HOME GAMES AT PLANTATION ONLY
All players must complete the Sign-Up two-step process by:
1) completing this sign-up form and submitting online and 
2) paying your weekly golf fee using Zelle to Hilene Bilchik at dreamcatchertt@gmail.com or 305-206-2003. 

Cost to Pay:
Premier card holders: $40.66 plus $5 for the game =  $45.66
Non-Premier Card:  $53.50 plus $5 for the game   =    $58.50
Plantation Preserve Members:  $26.00 plus $5 for the game = $31.00
