This form must be completed by a Parent or Guardian with the young person they are responsible for. It should take approximately 10-15 minutes to read and complete. Please ensure you have enough battery charge and a good internet connection before starting. Any information not submitted will be lost.  Some sections of the form are repetitive but are required to meet our GDPR obligations.

Following the completion of this form the Youth and Community Team leader of the chosen club will contact you to introduce themselves, to confirm your child's space in the club and to give you information regarding club rules and fees.
Introduction
The physical clubs offer the usual regular activities like air hockey, table football,  pool, arts and crafts etc. but with extra fun activities focusing on relationships, health and well-being including sexual health, personal safety, participation and independence.

Online clubs and sessions offer fun activities and 2 to 1 support.

New members will be given an induction to physical clubs settings covering rules, safety and safeguarding. They will be buddied up with a long standing member. The buddy will show them around, explain how the club works and where the toilets are and generally make them feel welcome.

New on-line members will be given a code of conduct briefing.

Members are asked for their opinion on how the club is run and activities are based on their interests.

We provide opportunities for young people to seek support and advice relating to health, personal issues, education and careers on an ‘established need’ basis.

All staff hold an enhanced clean DBS certificate.  They also receive regular training in safeguarding, health and safety, fire safety, first aid and safe food handling.

Because we need to reduce the numbers attending the youth club at any one time. Please state the preferred day, if your club offers more than one session a week. (Street / Frome)
If you don't live near any of the physical youth clubs listed above but you would just like to attend online clubs and support. Please place a tick in the yes box. Members of a physical youth club will automatically be invited to online session unless you opt out later in the form.
