Please fill out this form for EACH of your children at the middle school. It is vital that we know how your student will participate in school this fall.

This form is available from August 12 - August 18, 2020.

As we plan and prepare for the year ahead, the district is requiring that you notify us of your family’s decision to enroll your child in our Remote Learning Academy for grades 5-8, the hybrid model for grades 5&6 or the remote model for grades 7&8 no later than AUGUST 18th. Please know the DESE safety guidelines will be followed for all individuals in the building, including mandatory wearing of masks. Additionally, those students not enrolled in the Remote Learning Academy may move between hybrid and remote models throughout the school year.
