This is the form is for accessing neighbourly support throughout Heaton, Ouseburn, Battlefield and Shieldfield if you are in self-isolation, because you have or may have Covid-19, or are immunocompromised, diabetic, asthmatic, elderly or otherwise vulnerable. We hope this will help reduce the number of severe and critical cases of Covid-19, prevent a sharp peak of cases and relieve pressure on public services.

By filling in this form you are consenting for your information to be stored by the Heaton, Ouseburn, Battlefield and Shieldfield help for self-isolators group to help us track requests, and to be forwarded to the sub-group operating in your area, who may also store your information.
